You can schedule your initial consultation online or over the phone. In order for us to provide you with the best service, we will need your insurance information, date of birth, referring doctor, if applicable, and your email address. This will allow us to have all the information so that we can verify your benefits ahead of time as well as email your registration forms to complete before your appointment.
Please plan for approximately one and a half hours for your initial evaluation. Arrive 15-30 minutes prior to your evaluation in order to complete your intake paperwork. When you arrive, our office staff will review your benefits quoted from your insurance provider and collect any deductible, copay, or coinsurance. During your first visit, your therapist will conduct your initial evaluation to assess your concerns. Your initial evaluation will typically consist of discussing your medical history and current health, diagnostic testing, discussing the results and your plan of care, treatment, and your home-exercise program. A copy of your initial evaluation will be sent to your physician.

For your evaluation, you will meet with your physical therapist. They will talk with you about your entire past medical history and do a deep dive into your current condition including learning about all the activities that you cannot do or are having difficulty doing. Next, they will evaluate your current symptoms and perform diagnostic testing to find the root cause of your condition. From here, they will bring all that information together and formulate a personalized plan of care for you to get you on the road to recovery, get rid of your symptoms, and back to doing everything you love.
Typically your personalized plan of care will consist of multiple weekly appointments and designed to help you meet all your physical therapy goals. You will have a home exercise program too. If you are consistent with both your physical therapy plan of care and your home exercise plans you will have shorter recovery times and fewer recurrences of injury. Your treatment plan ensures progression to your goals in the most efficient amount of time.

Physical Therapy is a journey, that's why it's important to contact us right away if you feel your symptoms returning. Often times, we can help you with less visits, if you contact us immediately.
We always recommend you complete a 6 month follow up appointment as a preventative measure. So put a reminder in your phone on the last day of therapy.
